Questionnaires on Paper (English) with The ASQ User's Guide
Discover why users in the United States, Canada, and around the world have found ASQ to be the flexible, culturally sensitive, and economical way to screen infants and young children for developmental delays or concerns in the first 5 years of life. ASQ components include - 19 questionnaires and corresponding scoring sheets. Parents complete the 30-item photocopiable questionnaires at designated intervals. Professionals convert parents' responses of yes, sometimes, and not yet to the scoring sheets, enabling them to quickly determine a child's progress in each of 5 key developmental areas--communication, gross motor, fine motor, problem solving, and personal-social. Each questionnaire takes 10-20 minutes to complete and 2-3 minutes to score. Programs can use all 19 questionnaires or select the age intervals most useful to them. The questionnaires are in PDF format and can be printed or photocopied as needed, for no additional cost. The questionnaires can be posted on an internal office network or on a password-protected web site, and they can be used concurrently by multiple sites with the purchase of a site license. Bonus: the English CD-ROM has the intervention sheets from the ASQ User's Guide in PDF format, making them even easier to share with families; the Spanish CD-ROM has 200 intervention activities in Spanish never before included with any ASQ product. - ASQ User's Guide. The User's Guide helps professionals accurately administer the questionnaires and interpretthe results with the validation data included for the forms. Sample parent-child activities for each age range are included. - The Ages & Stages Questionnaires on a Home Visit. An easy way to train staff members, this 22-minute videotape shows a home visitor guiding a family with three children through the items on an ASQ questionnaire. - ASQ Scoring & Referral. This informative video provides an overview of ASQ and shows how to score the questionnaires accurately and make informed decisions about referrals. - Coming Soon! ASQ Manager. This computer database program helps centers and agencies use ASQ more efficiently. It creates an individual file for each child that includes contact information, demographic information, ASQ screening results, and notes. After users enter answers to ASQ questions into the child's record, the program tabulates scores, identifies areas in which the child needs further assessment, formats questionnaire information into a printable document for sharing with parents, and generates a letter to the parents explaining the results. The program can also generate reports of all children in the center, helping users identify which children need to be screened on a month-by-month basis, which children need follow-up assessment, and more. The ASQ Manager comes with a detailed user's guide and a brief, simply worded guidebook on developmental screening. |
